You have a right to access health care records that belong to you or your underage child.
Please submit a written request to your therapist or to info@bluehorizoncounseling.com. Include:
- Full Name
- Date of Birth
- Contact Information
- Type of records requested
- Timeframe of request

Payment and Insurance

Blue Horizon Counseling accepts many major Behavioral Health Insurance plans and Employee Assistance Programs (EAP’s):
- Aetna
- Blue Cross Blue Shield
- Cigna
- Magellan (for some therapists)
- Medicare
- United Behavioral Health
- If we are out of network, we can provide ‘superbills’ for you to submit and potentially get reimbursed.
All out of pocket expenses can be paid with cash or major credit cards, including Health Saving Accounts (HSA) and Flexible Spending Accounts (FSA).
What to Expect

It all starts with the first point of contact. We will get all of your relevant information and verify your benefits. Once your intake paperwork is complete, we will then contact you to schedule your first appointment and answer any additional questions.
At your first session, you will be warmly greeted by your therapist, who will create a comfortable and welcoming environment for you. During this initial meeting, you will be given a detailed description about the counseling process. This includes what to expect, the confidentiality measures in place, and the treatment options that may be employed during your journey. You will have the opportunity to ask any questions you might have, ensuring that you feel fully informed and at ease.
Additionally, your therapist will take the time to verify that you understand what’s in store for you, addressing any concerns that may arise. From there, you’ll begin to develop a meaningful relationship with your therapist, who will walk alongside you on your unique journey of healing and growth, providing guidance, support, and insight as you navigate the challenges ahead, and helping you to build resilience and self-awareness along the way.
Cancellation Policy

As a practice, we are conscientious of how valuable your time is. We respect your limited resources, including your time and money. With that in mind, we give you 100% during your session time.
Our other client’s and employee’s time are just as important. If you are unable to make your session, we require that you give us at least 24 hours notice. This allows us to open that time up for someone in need if you won’t make it. Not giving adequate notice (outside of emergencies) will result in a late cancellation/no show fee of $75 being charged to your account.
Good Faith Estimate

January 1, 2022, the No Surprises Act was enacted. It exists to protect consumers from surprise medical bills from healthcare professionals.
If you are not using insurance, you are entitled to a good faith estimate for your services. This ensures that you are not caught off guard with a large bill.
You are completely in control of the cost of your services, we will tell you upfront how much each session costs, and will determine the frequency and duration of therapy based on your finances. You are also able to discontinue services at your discretion, which gives you complete control of your finances.
HIPPA

HIPPA requires healthcare professionals to ensure confidentiality of our client’s Protected Health Information (PHI). This includes your relationship with Blue Horizon Counseling and the content of your sessions.
There are a few exceptions to what is protected: you give written permission (i.e. to bill your insurance company), you are a danger to yourself or others, you share information about child or elder abuse, or your therapist’s records are subpoenaed. Please ask if you have concerns as to how we address these concerns.
